PRINCIPLE
Direct (conjugated) bilirubin, in an acid medium, reacts with the diazonium salt to form a red azobilirubin compound. The intensity of the colour produced in the reaction is proportional to the bilirubin concentration in the sample and is quantified photometrically.
DIAGNOSTIC USE
Bilirubin is a breakdown product of hemoglobin that is transported to the liver together with albumin. There it is conjugated with glucuronic acid and is excreted through the bile. This conjugated bilirubin is called direct bilirubin, while unconjugated bilirubin is called indirect bilirubin. Total bilirubin in serum is equivalent to direct bilirubin plus indirect bilirubin. Increased direct bilirubin may be due to liver abnormalities, bile duct obstruction, cirrhosis, and hepatitis.A single test result could not be used to make a clinical diagnosis. It should integrate clinical and laboratory data.
